Prevention Program Activities Funded by the State Department of Health

Healthy Heart Program

Through Healthy Heart Program funding, the Glens Falls Hospital has worked in a variety of community settings to increase opportunities for physical activity and access to healthy foods. Interventions include:

  • Increasing healthful foods available at convenience stores;
  • Increasing walkability (e.g., sidewalks, trails, pedestrian flags) in towns and villages;
  • Training children in bicycle safety;
  • Providing WIC participants with physical activity resource bags;
  • Installing bike racks; and
  • Advocated for the inclusion of biking and walking in town plans.

Abraham Wing Elementary School in Glens Falls, NY ( Warren County)

The school has made changes to the school day by instituting recess before lunch, healthy school foods, encouraging walking to school, and biking to school after third grade when a bike training program is provided for every interested student. The school is in the process of beginning a school environment study related to Safe Routes to School in collaboration with local police, and the local Metropolitan Planning Organization to improve the numbers of walkers and the safety of walking around the school.

Warrensburg Central School in Warrensburg, NY ( Warren County)

The school has increased physical activity through the promotion of ropes courses and trikes in physical education. It has also conducted safe walking programs.

The Overweight and Obesity Prevention Program

The Foundation for Healthy Living

The Foundation for Healthy Living, located in Latham, NY, is a tax-exempt public charity organization created by HealthNow New York, Inc. to improve the health of people living in Upstate New York.

The Foundation received a five-year grant to establish a Center for Best Practices for the prevention of childhood overweight and obesity in young children. Preventing Childhood Overweight and Obesity Starts Early (CHOOSE), www.healthykidsny.org/ focuses on increasing community awareness of the public health threats of early childhood overweight and obesity and increasing obesity screening and counseling among health care providers in Albany, Schenectady, Rensselaer, Saratoga, Warren and Washington counties.

The Center partners with multiple organizations such as the Capital District YMCA, the American Dairy Association, Price Chopper, the New York State Dietetic Association and Blue Shield of Northeastern New York to develop a website, community resource guide and implement a widespread social marketing campaign.

The Center and its partners sponsor Continuing Medical Education (CME) events for physicians, nurses and dieticians and curricula to support healthy eating, increase physical activity and decrease screen time in child care centers.

Glens Falls Hospital in Glens Falls, NY ( Warren County)

The hospital received a one-year grant to implement the Coordinated Approach to Child Health (CATCH) Kids Club curriculum, make nutrition and healthy snack changes, and work with school food service personnel to analyze vending machine options in approximately 20 YMCA afterschool and summer day camp settings in Warren and Washington counties.

Diabetes Prevention and Control Program

The Seven County Diabetes Network, led by Glens Falls Hospital in Glens Falls, is one of 15 regional diabetes coalitions funded by the Diabetes Prevention and Control Program. The coalitions provide community-based programming for the prevention of diabetes and its complications. In addition to hosting educational events focused on nutrition, physical activity and diabetes management, the coalitions engage in education for health care providers working with people with or at risk for diabetes, and promote policy, systems, and environmental change. Examples of changes include expanding walking trails, including healthy choice foods and beverages in vending machines, and offering flex time for employees to take longer walks during their lunchtime.

Warren County is one of seven counties represented in this coalition’s catchment area. Partners include the Warren County Health Department, Glens Falls Hospital, and other community-based organizations. This coalition conducts diabetes prevention education programs for at-risk rural populations, partners with worksites to complete the Heart Check 4.1 worksite assessment tool, and develops policies that provide increased opportunities for healthy choices in workplace vending and food service areas.

Diabetes Prevention in Children

Glens Falls Hospital is one of eight Type 2 Diabetes Prevention in Children contracts funded by the Diabetes Prevention and Control Program. The project features partnerships with local schools to implement Girls on the Run, an internationally recognized evidence-based program that educates and prepares pre-teen girls for a lifetime of self respect and healthy living.

WIC

The Warren County Health Services administers the WIC program, at nine sites in Warren County serving over approximately 1,500 women, infants and children each month. WIC has implemented a number of obesity prevention initiatives, including breastfeeding support, Fit WIC physical activity training for parents, patient-centered nutrition education, low-fat milk promotion, and the new WIC food package which includes vegetables and fruits, whole grains, and non-fat and low-fat milk.

Increasing Access to Healthy Foods in the Emergency Food Network

Through the Hunger and Nutrition Assistance Program and the Just Say Yes to Fruits and Vegetables Nutrition Education Program, emergency food recipients have increased availability of healthy foods like fresh produce and 1% or fat free milk, and an increased knowledge of how to prepare these foods. Last year, more than $326,000 worth of fresh produce and 1% or fat free milk was made available to emergency food relief organizations in the Regional Food Bank of North Eastern New York region which covers 23 counties. In addition, 3,125 individuals participated in 249 nutrition education workshops in the Regional Food Bank of North Eastern New York region last year.
