About HCRA Provider List

The HCRA provider list which is updated monthly is a list of New York State health care providers that offer services subject to HCRA surcharges. This list is subdivided by provider type as follows: general hospitals, hospital-based extension clinics, comprehensive diagnostic and treatment centers, diagnostic and treatment center extension clinics, diagnostic and treatment centers that provide ambulatory surgical services, and clinical laboratories. Providers are listed alphabetically within provider type.

The Provider File contains the list of hospitals, comprehensive diagnostic and treatment centers, diagnostic and treatment centers that provide ambulatory surgical services, and clinical laboratories. Hospital-based Extension Clinics and Diagnostic and Treatment Center Extension Clinics are listed in separate files. These files may be downloaded for use in data processing applications.

The October 1, 2000, surcharge assessment exemption authorized under HCRA 2000 for freestanding clinical laboratory services has been approved by the federal government. Beginning with the provider list effective March 1, 2002, all freestanding labs that opened on or after October 1, 2000, have been removed.

Amendments to Provider List

April 30, 2007 - Ultimate Health Care Outcomes, Operating Certificate # 2912201R, was added to the comprehensive clinic provider list dated "As of August 1, 2004", posted in early September 2004. This facility was incorrectedly designated by the Department of Health as a comprehensive clinic. Ultimate Health Outcomes has been removed from the comprehensive clinic provider list dated "As of May 1, 2007." Any payors that have made surcharge payments for services rendered by Ultimate Health Outcomes as a result of this error should contact the Department of Health at (518) 474-1673 for further instructions.

March 24, 2006 - Celia Dill Dialysis Center, Operating Certificate # 3950201R, was erroneously added to the comprehensive clinic provider list dated "As of January 1, 2006", posted in early February. This facility was never designated by the Department of Health as a comprehensive clinic. Celia Dill has been removed from the comprehensive clinic provider list dated "As of February 1, 2006", posted in early March. Any payors that have made surcharge payments for services rendered by Celia Dill as a result of this error should contact the Department of Health at (518) 474-1673 for further instructions.

Users of the HCRA designated providers list should note that one provider included on the comprehensive clinics portion of the list, Wartburg D&TC, was added to the list in error. This facility, which opened on December 16, 1997, was never designated by the Health Department as a comprehensive clinic. Also, two other providers included on the comprehensive clinics portion of the list, Baker Victory Healthcare Center and Hudson Valley Health Specialties, have had their classifications as comprehensive clinics voided by the Department retroactive to their dates of inception. Therefore, patient services payments made to, or patient services revenue received by, these three providers for services rendered, are not subject to, nor should they have been subjected to, assessment of the HCRA surcharge.
