Registration in the New York State Organ and Tissue Registry is voluntary. The New York State Department of Health will only collect information you provide on the Registry enrollment form. The Department will be unable to accept your enrollment application unless all the requested information is entered on the form. Registry information is kept strictly confidential and can only be accessed by Department of Health employees when required for the performance of their official duties, and by federally regulated organ procurement organizations and New York State-licensed tissue banks and hospitals at the time of your death. The information submitted by you will be maintained in the Registry until you have requested in writing that such information be deleted, or until confirmation of your death. To change the information pertaining to you that is maintained in this Registry, please re-submit the online enrollment form.

You may obtain a copy of any personal information pertaining to you that is maintained in this Registry by writing to the address below. The request must include your name, address, date of birth and either your New York State Department of Motor Vehicles license number or non-driver identification number (if available).
