Nursing Home Transition and Diversion Waiver

The Department of Health is requesting comment on the design of a new Medicaid Nursing Home Transition and Diversion waiver from all interested consumers, providers and organizations. The RFI and a copy of the enacting statute, S7515, can be accessed on the DOH website.
