Diabetes Partnerships Around New York State: Diabetes Prevention and Control Program Contractors
The New York State Diabetes Prevention and Control Program (DPCP) support many different contractors throughout New York State.
Community Coalitions for Diabetes Prevention
There are 15 regionally-based Community Coalitions for Diabetes Prevention that, in partnership with community based organizations, work to expand, enhance and improve services for persons with diabetes or those at risk for developing the disease.
Coalitions include representatives from local health departments, hospital-based diabetes programs, Cooperative Extensions, and a number of diverse community-based organizations dedicated to health improvement in disparate populations. Coalitions have established objectives that will allow progress toward achievement of six outcomes.
These outcomes include:
- Coalition capacity-building and sustainability;
- Primary prevention -- Preventing diabetes in people who are at highest risk of getting diabetes;
- Secondary prevention -- Preventing complications of diabetes in those already diagnosed with diabetes;
- Professional education -- Education about diabetes prevention and treatment for healthcare providers;
- Raising public awareness of diabetes risk factors; and
- Policy, environmental and systems changes. – Changes in the workplace or community that allow people to more easily make physical activity or nutrition changes in their lives.

Type 2 Diabetes Prevention in Children
There are eight projects across New York State working to prevent type 2 diabetes in children. Their goal is to reduce risk factors for type 2 diabetes in children by increasing opportunities for physical activity and healthy food choices in home, school and community.
Diabetes Centers of Excellence (COE)
There are five Diabetes Centers of Excellence to serve as New York State's premier hospital-based diabetes specialty centers. The centers incorporate current science to create comprehensive approaches for the prevention, early diagnosis, and treatment of pre-diabetes and diabetes. They demonstrate the potential for hospital-based diabetes programs to make positive changes in healthcare practices, disseminate findings and contribute toward systems change.
New York Diabetes Coalition
The DPCP, in collaboration with the New York Health Plan Association, has been educating providers and patients on diabetes standards of care and self-management practices. Now, the techniques used are being tested in several private practices….The results of these tests will be used to improve the quality of diabetes care across the state. The DPCP, in collaboration with the New York Health Plan Association, is involved in an ongoing diabetes quality improvement project with the New York Diabetes Coalition (NYDC). The initial focus of the project was the education of providers and patients on diabetes standards of care and diabetes self-management practices. The DPCP is now working with the NYDC to test the replicability of the Care Model what is this? In several private practices in Westchester and Sullivan Counties. Lessons learned from this project will be shared with other diabetes stakeholders across the state to influence diabetes quality of care practices.
National Kidney Foundation
The DPCP, in collaboration with the National Kidney Foundation, is supporting a continuing education program on the relationship between diabetes and Chronic Kidney Disease (CKD) for primary care providers, cardiologists, and endocrinologists. The project will help improve care for diabetes patients by educating providers on the early evaluation and management of CKD. A series of Grand Rounds, developed by clinical experts, and based on current clinical practice guidelines will be "team taught" by endocrinologists and nephrologists.
